Introduction: Wounds subjected to mechanical tension are at increased risk of impaired scar quality. The combination of a skin tension-relief device with negative pressure wound therapy (NPWT) may improve load distribution and scar evolution. Objective: To compare scar quality in patients treated with a skin tension-relief device combined with NPWT versus a skin tension-relief device alone in high-tension reconstructive wounds. Methods: A prospective, longitudinal, quasi-experimental study was conducted in 20 patients aged 25 to 60 years. Ten patients received a skin tension-relief device alone, and ten received the device combined with NPWT. The observer component of the Patient and Observer Scar Assessment Scale (POSAS) was applied at 90 and 180 days. Results: The mean global POSAS score was lower in the combined therapy group at 90 days (7.70 versus 9.40) and at 180 days (6.20 versus 8.70). Descriptive differences favored the combined therapy group, particularly in scar thickness, pliability, and surface characteristics. No statistical significance tests or comparative counts of wound dehiscence were reported. Conclusions: In this small, non-randomized cohort, the combination of a skin tension-relief device and NPWT was associated with better descriptive scar quality scores.
