Introducción: La citohistopatología constituye un pilar fundamental en el diagnóstico oncológico, siendo la técnica de block cell una herramienta esencial para la preservación de la arquitectura celular y la realización de estudios inmunocitoquímicos. Objetivo: Determinar la utilidad de la dimetilacetona como agente fijador en la formación de block cell para el diagnóstico citohistopatológico de líquidos serosos y biopsias por aspiración con aguja fina (BAAF) tiroidea. Metodología: Estudio cuasiexperimental de tipo aplicativo con enfoque cuantitativo, en una muestra de 200 especímenes (90 líquidos pleurales, 70 líquidos ascíticos y 40 BAAF tiroideas) procesados en el Laboratorio CITOPAT durante el período julio-diciembre de 2024. Resultados: Se demostró una correlación positiva alta entre la dimetilacetona y la calidad del block cell mediante el coeficiente Rho de Spearman (rho=0,852; p<0,01), así como una excelente confiabilidad del instrumento (Alfa de Cronbach=0,968). La concordancia interobservador fue significativa en todos los ítems evaluados (p<0,01). Conclusión: La dimetilacetona es un fijador eficaz para la formación de block cell, superior a los métodos convencionales en la preservación del detalle celular.

Introduction: Cytohistopathology is a cornerstone of oncological diagnosis, with the cell block technique serving as an essential tool for preserving cellular architecture and enabling immunocytochemical studies. Objective: To determine the efficacy of dimethylacetone as a fixative in the cell block technique for the cytohistopathological diagnosis of serous fluids and thyroid fine-needle aspiration biopsies (FNAB). Methods: A quasi-experimental study was conducted using a sample of 200 specimens (90 pleural fluids, 70 ascitic fluids, and 40 thyroid FNABs) processed at the CITOPAT Laboratory between July and December 2024. Results: Findings revealed a high positive correlation between dimethylacetone and cell block quality (Spearman's rho = 0.852; p < 0.01). Instrument reliability was excellent (Cronbach's alpha = 0.968), and inter-observer agreement was statistically significant across all parameters (p < 0.01). Conclusion: Dimethylacetone is an effective fixative for cell block preparation, outperforming conventional methods in the preservation of cellular detail.
