Telemedicine in Prehospital Care: Use and Perception Among Emergency Personnel in Cuenca, Ecuador

Introduction: Telemedicine constitutes a relevant tool in contemporary healthcare systems, particularly in emergency medical services, where real-time communication and the transmission of clinical data can decisively influence service effectiveness. Objective: To describe the perception, current level of use, and identified needs for the implementation of telemedicine by prehospital care professionals in the city of Cuenca, Ecuador. Methodology: An observational, descriptive, cross-sectional study was conducted using a self-administered digital survey. Sixty professionals from public and private institutions linked to prehospital services participated. The instrument assessed general knowledge about telemedicine, usage experience, technological availability, received training, perceived barriers, and future willingness. Results: It was found that 56.7% of participants were between 25 and 34 years old, with a male predominance (56.7%). 85.0% considered the available prehospital resources to be insufficient, and 61.7% rated the institutional technological infrastructure as moderate. 78.3% recognized the concept of telemedicine, although only 51.7% reported using it occasionally. 55.0% of the institutions lacked remote monitoring equipment, and only 10.0% of the personnel considered themselves fully trained. Conclusions: Prehospital personnel in Cuenca exhibit a favorable attitude toward telemedicine, although significant gaps persist in infrastructure, equipment, and training. These findings justify the design of institutional strategies that articulate technological investment, continuous training, and specific operational protocols to consolidate telemedicine as an integral component of the prehospital emergency system.
Introducción: La telemedicina constituye una herramienta de relevancia en los sistemas de salud contemporáneos, particularmente en los servicios de emergencias médicas, donde la comunicación en tiempo real y la transmisión de datos clínicos pueden influir decisivamente en la efectividad del servicio. Objetivo: describir la percepción, el nivel de uso actual y las necesidades identificadas para la implementación de la telemedicina por parte de los profesionales de la atención prehospitalaria en la ciudad de Cuenca, Ecuador. Metodología: Se realizó un estudio observacional, descriptivo y transversal mediante una encuesta autoaplicada en formato digital. Participaron 60 profesionales de instituciones públicas y privadas vinculadas a los servicios prehospitalarios. El instrumento evaluó conocimientos generales sobre telemedicina, experiencia de uso, disponibilidad tecnológica, formación recibida, barreras percibidas y disposición futura. Resultados: se obtuvo que el 56,7% de los participantes tenía entre 25 y 34 años, con predominio del sexo masculino (56,7%). El 85,0% consideró que los recursos prehospitalarios disponibles son insuficientes y el 61,7% calificó la infraestructura tecnológica institucional como moderada. El 78,3% reconoció el concepto de telemedicina, aunque solo el 51,7% declaró utilizarla ocasionalmente. El 55,0% de las instituciones carecía de equipos de monitoreo remoto y apenas el 10,0% del personal se consideró plenamente capacitado. Conclusiones: El personal prehospitalario de Cuenca exhibe una actitud favorable hacia la telemedicina, aunque persisten brechas significativas en infraestructura, equipamiento y capacitación. Estos hallazgos justifican el diseño de estrategias institucionales que articulen inversión tecnológica, formación continua y protocolos operativos específicos para consolidar la telemedicina como componente integral del sistema de emergencias prehospitalarias.
