Pernicious anemia as a manifestation of autoimmune polyglandular syndrome type 3B: a case reportContenido principal del artículo
Introduction: Autoimmune polyglandular syndrome type 3B (APS-3B) is a rare condition characterized by the coexistence of autoimmune thyroid disease and immunological gastrointestinal or hepatic disorders, specifically in the absence of Addison's disease and hypoparathyroidism. Its diagnosis necessitates a comprehensive clinical evaluation due to the wide spectrum of associated manifestations. Objective: To describe a case of autoimmune polyglandular syndrome type 3B by integrating clinical, hematological, immunological, and endoscopic findings, emphasizing the importance of a multidisciplinary approach. Methodology: A clinical case report is presented involving a 35-year-old female with severe macrocytic anemia, critical vitamin B12 deficiency, hyperhomocysteinemia, and extensive mucosal lesions. The diagnostic workup included hematological and immunological studies, alongside endoscopic procedures, to establish the link between clinical manifestations and the underlying autoimmune disorder. Results: The integration of clinical findings confirmed the diagnosis of APS-3B. Treatment consisting of parenteral vitamin B12 replacement and structured endocrinology follow-up led to a favorable clinical outcome, characterized by the progressive healing of mucosal lesions and the normalization of hematological parameters. Conclusions: Early recognition of APS-3B requires a multidisciplinary assessment to identify complex autoimmune associations. Adequate vitamin B12 supplementation and endocrinological monitoring are fundamental pillars for improving prognosis and preventing complications.
